William Wheeler Woodworth: 64^th Regiment, NY State Volunteers. Age, 41 years. Enrolled at Elmira, to serve three years, and mustered in as 1^st Lt., Company D, December 10, 1861; promoted Captain, February 26, 1862; died of disease, December 28, 1862, near Falmouth, VA.

Commissioned 1^st Lt., December 2, 1861, with rank from November 27, 1861, original; Captain, March 6, 1862, with rank from February 26, 1862, vice Phipps Lake resigned.

He is listed as Woodworth, Wm. H. in the Adjutant General's Report.
